What Is PT-141?
PT-141, generically known as bremelanotide, is a synthetic cyclic heptapeptide derived from the hormone alpha-melanocyte-stimulating hormone (α-MSH). It was initially developed from Melanotan II and was investigated as a tanning agent before its sexual health effects were discovered. PT-141 activates melanocortin receptors (MC3R and MC4R) in the central nervous system — specifically in areas governing sexual arousal and desire — making it one of the only sexual health medications that works centrally rather than peripherally.
How PT-141 Works
Unlike PDE5 inhibitors (Viagra, Cialis) that dilate blood vessels to improve erectile mechanics, PT-141 activates neural pathways in the hypothalamus associated with sexual motivation and reward:
- Central mechanism: Acts on MC3R and MC4R receptors in the brain, not peripheral vasculature
- Desire-focused: Increases sexual motivation and arousal at the psychological level
- Works in both sexes: Effective for women (HSDD) and men (libido-driven ED)
- Independent of hormones: Can be effective even when testosterone levels are normal
- On-demand: Taken 45 min–4 hours before activity; not a daily medication
Who Is PT-141 For?
Women
FDA-approved for hypoactive sexual desire disorder (HSDD) in premenopausal women. Also used off-label for post-menopausal sexual dysfunction, arousal disorders, and as a complement to hormone therapy.
Men
Used off-label for erectile dysfunction with a desire/libido component, as an adjunct to TRT for men whose primary complaint is reduced drive, and for situational performance issues. Particularly useful when PDE5 inhibitors don't address the root issue.
Administration & Dosing
PT-141 is administered by subcutaneous injection (typically abdomen or thigh):
- Timing: 45 minutes to 4 hours before sexual activity
- Standard dose: 1.75–2mg per use (physician may adjust based on response and tolerability)
- Frequency: On-demand; not recommended more than once in 24 hours or more than 8 times/month
- Duration of effect: 6–12 hours
To minimize nausea (the most common side effect), many patients start at a lower dose (1mg) and titrate up. Taking the injection in the evening and eating a light snack beforehand also helps.
Safety Profile
PT-141 has an established safety record from its FDA approval process and clinical use. The most common side effects are nausea (dose-dependent), facial flushing, and injection site discomfort. Rare cases of transient blood pressure elevation have been reported. It is contraindicated in patients with high cardiovascular risk.
